What hyperhidrosis is

Hyperhidrosis means sweating far more than your body needs to cool itself. It isn’t about being out of shape or anxious. In primary hyperhidrosis, the nerves that switch on your sweat glands are overactive, often from the teenage years on, and it tends to run in families.

It shows up in small, constant ways. You plan outfits around sweat marks. You keep your arms pinned to your sides in meetings. You go through shirts, deodorants and clinical-strength antiperspirants, and none of them hold up through a hike up the M, a day at Bridger, or a warm July afternoon downtown.

How we treat it at Instinct

We use RF microneedling on the underarms. Very fine needles deliver radiofrequency energy into the layer of skin where the sweat glands sit. That controlled heat reduces the activity of a portion of the glands, so the area produces less sweat.

The device we use is FDA-cleared for RF microneedling. Using it to treat sweating is an off-label use, meaning it’s a use chosen based on published research rather than one named in the device’s clearance.

The research is encouraging. In a sham-controlled study, people had three sessions on one underarm while the other was treated with the device switched off, and only the treated side improved. Follow-up found the effect held for about a year for many people, which is why we recommend a yearly maintenance session. The studies are still fairly small, and results vary from person to person.

Treatments are performed by a trained provider under the supervision of our medical director, Dr. Sarah Donahue.

What to expect

Consultation. We confirm your sweating pattern and rule out anything that should be seen by a physician first.
Arrive without numbing cream. Please don’t apply numbing before your appointment. We start with an iodine sweat test, which shows exactly where you sweat so we can map the right treatment area.
Numbing. Once the area is mapped, we apply topical numbing to the underarms. It takes about 40 to 60 minutes to take effect.
Treatment. The treatment itself takes about 30 minutes. Most people feel warmth and pressure. Plan on about 1.5 to 2 hours for your whole visit.
Aftercare. Expect redness, swelling and tenderness for a few days. Skip deodorant and antiperspirant for 72 hours after each session.

This treatment reduces sweating. It doesn’t switch it off completely, and you wouldn’t want it to. Results build over the series, and most people need a maintenance session about once a year to keep them. We’ve only recently started offering it, so we’ll be honest with you at your consultation about what to expect and whether you’re a good fit.

Who it's for

– Adults with heavy underarm sweating that clinical-strength antiperspirants haven’t controlled
– People who want a non-injection option
– People whose sweating started young and has been steady for years

See your doctor first if your sweating started suddenly in adulthood, happens all over your body rather than in one area, comes with night sweats, fever, weight loss or a racing heart, or started after a new medication. Some sweating is a sign of something else.

This treatment also isn’t right for you if you’re pregnant or breastfeeding, have a pacemaker or other implanted electronic device, have an active infection in the area, have uncontrolled diabetes, or are taking isotretinoin.
